Request for Hurricane Prep-Assistance Form

If you need assistance preparing your home in the event of an oncoming hurricane, you must fill out the Request for Hurricane Assistance form as soon as possible. Click here to download a form. The forms are also available at the back of the Worship Center on Sundays or at the Church Office. We have limited resources but will do our utmost to accommodate requests!
